Children’s Long-Term Support Waiver (CLTS)
What is CLTS?
The Children’s Long-Term Support (CLTS) Program is a Wisconsin Medicaid waiver program that provides services and supports for children with disabilities and their families. The program is designed to help children live their best lives at home and in the community.
Qualifications:
The waiver supports children who have limitations in daily activities as a result of one or more of the following disabilities:
-Developmental disabilities (DD)
-Severe emotional disabilities (SED)
-Physical disabilities (PD)
-Combination of the above
CLTS Services

Health & wellness
Services that help maintain or improve the health, wellbeing, socialization,
and inclusion of the child with their family and peers in the community.
Examples:
-yoga
-meditation
-health education
Support for parents and caregivers to learn new skills and boost
the child’s development and
independence.
Examples:
-caregiver mentoring
-resource materials

Caregiver supports & services

Daily living skills
Help for the child to build skills so they can do everyday activities by
themselves.
Examples:
-personal hygiene
-food preparation
-money management
